Proof of Address and Photo ID Information

To process your request for a letter, the practice requires proof of address (such as a tenancy agreement, council tax bill, utility bill, or bank statement) and photo ID (such as a passport or driving licence). Please ensure you attach both documents. Our practice policy allows up to 3 weeks (excluding weekends) to complete routine letter requests. Please note that we must receive proof of ID and payment before we can release the letter.
