Summary Care Records Policy

Your patient record is securely stored and kept confidential within the electronic system at our GP practice. If you need treatment at another NHS facility, such as an Emergency Department or Minor Injury Unit, sharing relevant information from your GP records with those treating you can help ensure you receive the most appropriate care.

This information can now be shared electronically via: The Summary Care Record, used nationally across England.

The information will be accessed only by authorized healthcare professionals directly involved in your treatment. Your consent will be requested before accessing your records, unless there is a clinical need and you are unable to provide consent.

If you would like to opt out, please fill out our Opt – Out form.

A parent or guardian can request to opt out a child under 16, but the final decision rests with the GP, who must consider their duty of care to the child. If you are a parent or guardian and believe your child is capable of understanding, you should share this information with them.
